During your first visit, 4 sponsorship cards will be given to you. You only have to pass them on to the people of your choice, who will give them to me the day of their first visit.

 

The sponsored person must not be a patient. Each sponsored person can only be sponsored once but can sponsor in his/her turn. The sponsor must have 4 sponsored people to enjoy the offer.
